End of Season Evaluation

The Big Lake Baseball Board of Directors take our responsibility seriously to provide a quality youth baseball program for our community, and continually seek ways to improve our program. Now that the current season is over, we would like to solicit your feedback concerning you and your child’s experiences this past season. The primary purpose of this brief evaluation is to assess our coaches’ performance, so we can continue to improve our coaches training and provide our coaches with every necessary means to succeed. Please keep in mind that this is an evaluation of your child’s current Head Coach only. Use the comment section at the end of the evaluation to give us feedback for anything else on your mind in regards to Big Lake Baseball. Positive feedback is always welcome! Please note that your response will remain private (even though your name is required on the form to maintain credibility).
